Fresh vs Cryopreserved Stem Cells — What Patients Need to Know

The honest difference between fresh and frozen stem cells, what the evidence says, and what matters more.

'Fresh cells' is one of the most heavily marketed phrases in Thai regenerative medicine. It is a real distinction, but it is not the distinction that most affects your outcome.

What the terms mean

Fresh — cells are cultured and administered without ever being frozen, usually within hours of harvest from culture.
Cryopreserved — cells are frozen in a cryoprotectant, stored, then thawed and washed before use.

What the evidence says

Freeze-thaw does reduce viability, typically by a measurable margin, and a thawed product must be washed and re-counted before administration. A well-run laboratory reports post-thaw viability on the certificate of analysis. In published comparisons, post-thaw MSC preparations that meet viability thresholds behave comparably to fresh preparations. The far larger variable is how many viable cells you actually receive, not whether they were once frozen.

What to ask either way

How many viable cells will I receive, and what is the viability percentage at the point of administration?
Can I see the certificate of analysis before I pay the balance?
Was sterility and endotoxin testing performed on this batch?
If cryopreserved — is the product washed after thaw to remove DMSO?
